To ensure we provide the best care possible, we kindly request that all new clients follow our streamlined procedure for scheduling appointments. Please carefully follow the steps outlined below:

Step 1: Provide New Client Information

As a new client, we kindly ask you to provide us with some basic information about yourself and your pet. Please fill out our New Client Information Form located here.

You can submit this information through our secure online portal, or by printing out the Client Information Form and emailing it to [email protected], or faxing it to 518-244-3290.

Step 2: Submit Previous Records

If your pet has received medical care from another veterinary clinic in the past, please submit their previous medical records to us. This information is crucial for our veterinarians to have a comprehensive understanding of your pet's health history. These records may be emailed to us at [email protected], or faxed to 518-244-3290.

Step 3: Receptionist Confirmation and Scheduling Fee

Once we have received both the previous records and client information form, our receptionist will review your submission. If everything is in order, they will contact you to schedule your appointment, and collect the scheduling fee.

Please note that the scheduling fee is required to secure your appointment slot. It applies to your first visit as a new client, and subsequent appointments following your first visit do not incur this fee. The scheduling fee covers the cost of the exam and is non-refundable if the appointment is canceled without 24 hours notice. The scheduling fee is also non-refundable if you are a no-show to your scheduled appointment.
